Will your health care decisions be followed if you are unable to speak for yourself because of a medical emergency? Roseville’s Community Health Awareness Team (CHAT) invites you to learn about Advanced Care Planning (ACP).
CHAT is co-sponsoring the free panel discussion and workshop: Advance Care Planning: Make Your Choices Known on Saturday, Oct. 15 from 10:30 a.m. to 12:30 p.m., at the Roseville branch of Ramsey County Library, 2180 Hamline Avenue N.
The discussion will feature a lawyer, a physician, and a Roseville firefighter/EMT who will offer practical insights into why Advance Care Planning is important in their work and why it should matter to you. Attendees will learn how to make sure they have a voice in their future health care decisions, even when they can’t speak for themselves.
Small group discussions led by trained Advance Care Planning facilitators experienced in helping individuals translate their wishes into legal health care directives will follow the panel discussion.
The Mitchell Hamline Law School mobile law network WHEELS OF JUSTICE staffed by attorneys and law students able to provide assistance on advance care planning and completing the health care directives (witnessing, notarizing, and counseling about distribution) will be available from 12:30 to 3:30 p.m. in the library parking lot following the discussion.
WHEELS OF JUSTICE is part of the Law School's efforts to reach out to communities to increase access and awareness of legal services. This service is perfect for young adults who want to complete the short form of the health care directive and for anyone who wants to update their existing directive.
