A health care home is an approach to primary care in which clinicians, families and patients work together to improve health for everyone, especially people with chronic health conditions and disabilities. Representatives from the Minnesota Dept. of Health, Entira Roseville/Como clinic and Multicare Associates - Roseville clinic will discuss how this reorganization is improving care delivery.
Health Care Home: You & Your Primary Care Team is the second event of In the Know: Managing Your Healthcare, a three-part series designed to provide essential information about vital healthcare topics. The series is sponsored by the Ramsey County Library, CHAT, and the City of Roseville.
